The Hazardous Waste Generator’s Exception Report is now on-line!

We’ve made the exception report process a bit easier by posting an e-form version of the Hazardous Waste Generator’s Exception Report PDF on-line.  Use this form when you have not received a manifest back from the designated facility within 35 days of the initial shipment. The generator must file the exception report with the department within 45 days of the initial shipment. Simply print the form and follow the instructions on the second page for filling it out.
